- Set goals for individual (weekly, monthly, 6 month, etc)
- Nutrition Program- 6 or 12 months: Review 2-week food log client will complete and create nutrition program together.
- Weight Loss Goal Setting (in conjunction with physical activity & training by Coach Brix). Goals will be individualized based on a person’s risk factors. (family history, weight, comorbidities, lab work, physical activity, etc)
- Medication Consultations & Recommendations
Laboratory Assessments and Monitoring - Diabetes Prevention Program
- Diabetes Self-Management Education
- Chronic Disease Education
Book Call with Coach Brix
- Discuss what the expectation are of the individual and myself.
- Set goals for the individual (weekly, monthly, 6 month etc…).
- Workout Program 2, 3 or 4 day a week program.
- Motivation talk to keep the client motivated to impact their lives.
- Any past injuries or problems.
- Limitations they may have.
- Video exercise description and go over how the program will work.
- Send video and workout program to the individual.
- Follow up with a 5-10 minute call that they received the program and understand how the program works.
